Crackup Sisters Videography and Photography waiver and consent form
I, the undersigned, or as the parent/legal guardian of the child named below, give my unreserved permission for all still and moving images taken or recorded by, on behalf of, or made available to the Crackup Sisters of myself / the below named child (“the Images”) to be:
Used in any or all promotional and advertising material of the Crackup Sisters; and/or
Provided to third parties, including but not limited to media organisations and Crackup Sisters partners, for their use.
The Images may be used in various formats including online media, social media, print, newspapers, video, public displays, television, and other electronic means, including edited forms.
No Compensation
I waive any rights and claims, present and future, to any fees, royalties, or other benefits whatsoever in connection with the use of the Images.
Withdrawal of Consent
If I wish to withdraw permission, I must notify the Crackup Sisters in writing.
I understand that:
The Crackup Sisters will cease future use of the Images where reasonably practicable; however
The Images may continue to appear in materials already produced or published.
Aboriginal and Torres Strait Islander Cultural Acknowledgement
(For Aboriginal and Torres Strait Islander persons)
I understand that Images may appear in printed and electronic materials for several years.
If I identify as Aboriginal and/or Torres Strait Islander, the Crackup Sisters will take reasonable steps to prevent use of the Images after my death. However, I acknowledge that some materials may continue to be circulated despite these efforts.
Child Safety and Protection Statement
The Crackup Sisters are committed to the safety, wellbeing, and dignity of all children involved in their activities.
In accordance with Queensland child safety expectations, I acknowledge and agree that:
Consent for a child’s participation and imaging is provided by a parent or legal guardian.
Images of children will be used in a respectful and appropriate manner and will not be used in a way that is exploitative, harmful, or inconsistent with community standards.
Personal identifying information (such as full names, locations, or contact details) will not be published alongside Images of children without additional consent.
Images will not be used in a way that could reasonably place a child at risk.
The Crackup Sisters will take reasonable steps to ensure that any third-party use of the Images aligns with appropriate child safety standards.
I understand that while reasonable safeguards are in place, once Images are published (particularly online), complete control over distribution cannot be guaranteed.
